Unwanted Balance Deduction Without My Permission For Insurance Premium By SBI

hi, on 20/12/16 rs 100 was debited from my account for insurance premium without my permission. This is third consecutive year I have been facing this problem.last er and this er I submitted application not to deduct any amount without my permission for insurance in future n requested them to refund it.last er they refunded but not this time. they did not inform me anything abt it and if something happens to me my family will not get benefited because no soft copy or hardcopy of insurance policy is been provided. this is one type of mental harassment i had been suffering. same policy is provided by govt of India at Rs 12 yearly. Yet bank officials are giv en extra burden on us. plz how can this problem been solved out plz advice me u had two receive copy of my application
