My grandfather have 1 Agri land of 7 bighas and his brother also has 7 bigha Agri land. The issue is his land is on my grandfather's name and my grandfather's land is on his brother's name. We want to exchange ownership of land and need to add my father's name to land ownership after said issue resolves. What will be the procedure for this?

The simple and proper method is to get an Exchange Deed executed between your grandfather and his brother in a very proper and legal manner and getting the document registered and get the required amendments made in the revenue records. Yes you can execute a exchange deed.. There is registration for exchange deed.. Either the Gfs or their legal heirs to execute. if more than one heir then other sibling of ur father has to relinquish their rights..
